Company check acceptance form	Customers who prefer to pay with comapny check need to complete this form, before submitting payment.
Credit Card Authorization Form	Authorization for N.E.M. to charge customer's credit card for products and shipping charges associated with you order. Customer will be notified on the amount he is being billed.
Credit Card Authorization Form 1	Authorization for N.E.M to charge customer's credit card for products and shipping charges associated with you order. Total charges will be given to customer in order to complete the form.
